Locums Dentist assignment in California - Located in Crescent City, CA - Dates: ASAP - Ongoing - Scheduled Clinical Hours Only - Outpatient Setting - Epic EMRRequirements: - Board Eligible - CA License - BLSDescription/Other Details: "Mon-Friday 8-5, Mon-Thur. 7-6, or Tue-Fri 7-650/50 kids/adultsThe provider must be willing to work 3 months minimum with the option to extend. No exceptions.Chart closure within 72 hoursA decision is typically made within 7-14 daysActive DEA needed before startCredentialing takes 30-60 days (six weeks is standard for Locums who submit app./documents timely)The following immunizations are required for credentialing (before start date): Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years).
