Description Supervisor Case Management for Providence Medford
Medical Center in Medford, OR. Full-Time/Day Shift The Supervisor
position reports to the Manager, Case Management and is responsible
for the direct supervision of the assigned Case Managers,
Coordinators, and Case Management Associates. Supervision includes
the daily coordination and management of all assigned Case
Management staff to ensure excellence in customer service. The
Supervisor develops and implements an operational plan that
includes staffing resources, policy and procedures, orientation and
education, and quality improvement activities under the direction
of the Manager and Director of Care Management. The position
assures timely provision of coordinated patient care that includes
all case management functions of care coordination, regulatory
management, discharge planning, and interventions through
application of case management standards of care. The Supervisor
maintains expertise in case management and ensures that services
are aligned with the vision and goals of the department. The
Supervisor ensures the development plan of individual and
professional group case management staff and ensures that services
are aligned with the vision and goals of the department. Providence

them. Required Qualifications:
- Bachelor's Degree in Nursing or
- Master's Degree in Social Services or equivalent
educ/experience
- Upon hire: Oregon Registered Nurse License
- 3 years Discharge planning and case management in an acute care
setting
- Working knowledge of MS Office Suite, including but not limited
to Word, Outlook, and Excel Preferred Qualifications:
- Master's Degree in Nursing or Community Health
- Supervisor or lead experience
- Crucial Accountability Training
