Join our compassionate Adult Intensive Services team at Klamath Basin Behavioral Health (KBBH), where you'll work alongside a multidisciplinary team to provide high-quality care to clients with diverse mental health needs.Key Responsibilities:
  • Patient Care & Support: Collaborate with psychiatric providers and clinical therapists to deliver exceptional care, including assessing and documenting vital signs, medication responses, and side effects.
  • Client Education: Educate patients and their families on medications and care plans, ensuring they fully understand treatments.
  • Coordination & Communication: Serve as a key liaison between providers, clients, community partners, and medical specialists to ensure seamless coordination of care.
  • Clinical Support: Manage medication samples, maintain records, and provide basic nursing care for minor illnesses or injuries, supporting other KBBH programs as needed.
  • Case Management: Coordinate with primary care providers, hospitals, rehabilitation programs, and other services to support clients in achieving the best health outcomes.What Success Looks Like:
    • You'll provide high-quality clinical support to psychiatric providers and clients, ensuring clear communication and smooth coordination of care.
    • You'll educate and empower clients and their families by explaining medication and treatment plans clearly and compassionately.
    • You'll take initiative in collaborating with other healthcare providers to improve client outcomes and maintain a seamless care experience.
    • You'll handle administrative tasks efficiently, from scheduling to documentation, ensuring organized and effective operations.Qualifications:
      • Active LPN license in the state of Oregon.
