We would try to place the RN Care Manager on a unit where they have expertise and keep them there as consistently as possible so they develop relationships with the care team and have continuity of assignment--- Will position float between units: Yes--- Is on-call required? No--- Are weekends required? Yes occasional weekends as needed--- Are block schedules required? No--- What are expected ratios? 1:17--- Special requirements: assists with durable medical equipment, home health and multiple types of post-acute placements such as Skilled Nursing Facilities, adult foster homes, and assisted living. works with multiple community partners to arrange plans and secure authorization from insurance companies.
